Program Description

The Santa Clara County 24-Hour Care Program is a centralized entry point for individuals entering in the Institute for Mental Diseases (IMD), Skilled Nursing Facilities (SNF), State Hospital, Private Hospital and Supplemental Services Program​. The role of 24-Hour Care Program is to provide psychosocial assessments for adult and older adult clients that are in acute hospitals to determine appropriate placement for long term psychiatric and community care placement following acute care in an IMD/SNF. 24 Hour Care also reviews request for designated behavioral health beds at shelters throughout Santa Clara County.

Referral Process

This program accepts referrals through the Behavioral Health Call C​enter​​. For more information about this program, or to see if you qualify, please contact the Behavioral Health Call Center at 1-800-704-0900.

Behavioral Health Urgent Care (BHUC)

871 Enborg Court, San Jose, CA 95128

BHUC services are temporarily relocated to the Central Wellness and Benefits Center building at

2221 Enborg Lane, San Jose CA.

Hours: Monday - Sunday, 8:00 a.m. - 7:00 p.m.

 

If this is a crisis or emergency call 911, and ask for a “CIT officer” (Crisis Intervention Team).

 

The Santa Clara County Adult and Older Adult Crisis Continuum are as follows: 

A 24-hour, toll-free confidential phone line to help people in crisis, give emotional support, suicide prevention, crisis intervention, and referrals to resources

Consists of a behavioral health professional riding with a Law Enforcement Officer

Crisis response services to individuals experiencing mental health crisis 

  • Crisis Stabilization Unit (CSU)

Services such as screening, assessment, treatment plans, brief therapy, peer support, to people and families, to stabilize a crisis 

Walk-in crisis clinic, with a psychiatrist on duty, for those who need voluntary services

24-hour locked psychiatric emergency room, which provides emergency psychiatric care

  • Crisis Residential

Alternative to inpatient hospitalization; provides therapy services in a protected, supervised, residential setting
