Learning Together Initiative - $ 4,289,271
A place-based approach focused on the provision of prevention, early intervention, and information to parents and community members to prepare children to be healthy and ready to enter and succeed in school. The information presented to families includes: health, school readiness, and early literacy/oral language development. The program has two primary components:
- Community Workers, community-based staff and volunteers focused on outreach and education to the broader community; and
- Family Resource Centers – high-quality family resource centers integrate health, education, and arts enrichment to promote school readiness, early literacy and oral language development, and overall well-being of children and families.
Services are provided by Mexican American Community Services Agency, Silicon Valley Education Foundation, Catholic Charities, Somos Mayfair, International Children Assistance Network , Choices For Children, Gardner Family Care Corporation, Indian Health Center, Sacred Heart Community Service, The Health Trust, SJB Child Development Center, Community Health Awareness Council, Jewish Family & Children's Services, City of San Jose Family Resource Centers.
Kid Connections Network of Care - $2,744,638
A partnership between FIRST 5 Santa Clara County, Santa Clara County Mental Health Department, the Inclusion Collaborative and a number of community-based organizations dedicated to supporting the healthy development of children ages prenatal through 5 years. Through the Mental Health Department Call Center and a network of Regional Assessment Centers, Kid Connections Network of Care provides access to developmental screening, assessment and early intervention and intervention services as warranted.
Services are provided by Mental Health Dept, Alum Rock Counseling Center, Catholic Charities, Children's Health Council, Community Solutions, EMQ FamiliesFirst, Gardner Family Care, Corporation, Kidango Inc, Rebekah Children's Services, Via Services, Parents Helping Parents.
