The Santa Clara County Behavioral Health Services Department offers a wide range of behavioral health services to ensure that our residents of Santa Clara County have access to help when they need it. The Behavioral Health Services Department contracts with various community-based organizations that provide outpatient support for adults, older adults, adolescents, children, and families. County and contract agencies are located throughout Santa Clara County. Below are the lists of programs sorted by County regions: North, Central, West, and South.

Caminar/Family & Children Services of Silicon Valley, previously Adult and Child Guidance center, provides high quality, affordable counseling, therapy and other support services in eight languages (including Spanish, Vietnamese, Cambodian and American Sign Language). Services include case management / brokerage services, money management, crisis intervention, individual, group, family therapy, assessments, evaluations, medication support services, and rehabilitation services. Services are provided in a variety of settings, including home, clinic, and community agencies as necessary for seriously mentally ill adult clients whose level of functioning, symptoms, and psychiatric history necessitate service intervention. The goal of the program is to maintain clients in community settings within context of the individual’s family, culture, language and community. The diagnostic spectrum includes schizophrenia, major depression, affective disorders, post-traumatic stress disorder, and the dual diagnosed mental health, developmental disability or drug and alcohol related diagnosis.
Specialized Services: The program also provides mental health service for Deaf & Hard of hearing mentally ill clients. Program services emphasize treating the mental health problems of abusive and multi-problem families to improve the mental health symptoms and dysfunction of deaf adults and their families. Treatment for the deaf and hearing impaired of any age are available in American Sign Language, with a TDD telephone for scheduling and information. Clinic hours are Monday-Thursday, 8:00 a.m. to 8:00 p.m. and Friday 8:00 a.m. to 5:00 p.m.

The Downtown Behavioral Health has three full-time Service Teams operating daily, Monday through Friday, 8am to 5pm. The Teams provide a full array of behavioral health services, case management services, crisis intervention, and medication support services. All Teams are comprised of care managers and a psychiatrist. The aim of each Team is to work with clients suffering from serious mental illnesses who exhibit severe problems in normal daily functioning. The goal is to assist individuals within the context of a mutual partnership effort to achieve higher levels of functioning, to develop community and/or family support systems wherever possible, to promote self-reliance and self-sufficiency, and to encourage individuals to work or to return to work whenever possible.

Gardner Family Care Corp. provides outpatient mental health services to predominately low-income children and families, adults and older adults. Services include a full array of mental health services: individual, couples family and group therapy; mental health assessments, neuropsychological assessments, home health assessments, psychological assessments, rehabilitation, case management, medication evaluation, injections and medication monitoring. Services are provided in the language of the target population, by culturally proficient staff, geographically accessible at the clinic and off site in key community location such as home, school and churches.

Grace Community Center provides accessible and adaptive recreational programming to adults with serious mental illness and developmental disabilities through its drop-in recreational center, weekly classes, meal program, day rehabilitation and support services.
The classes, day rehabilitation program and support services serve to develop important life skills that contribute to their sustainability in the community and provide productive activities that are fun and reduce social isolation. They are also designed to enhance coping skills that aid in the effective management of psychiatric symptoms.

The Mental Health Clinic at Juvenile Hall is an on-site intensive outpatient clinic, which provides multi-disciplinary, culturally sensitive behavioral health services to youth incarcerated in Juvenile Hall. LifeMoves, San Jose's major shelter provider in Santa Clara County, operates ten (10) programs each, targeting a unique homeless population. Its mission is to empower individuals to become self-sufficient in order to attain a stable living environment. Julian Street Inn provides semi-supervised emergency shelter/housing for SMI homeless adults, including meals, clothing, telephone/mail service, and linkages to financial and medical benefits and referrals to mental health service teams. LifeMoves operates nine (9) other homeless programs for men, women, and children.

Ujima Youth Program offers various afro-centric services targeting African American families and youth at risk. We provide 24-hour home-based case management services to those who are involved in the juvenile justice system, and African American Survival Skills Groups for youth and families. East Valley Behavioral Health provides services to East San Jose and Milpitas from the site of the East Valley Health Center at McKee and Jackson. There are three services teams, each providing a full range of outpatient services: medication, case management, crisis intervention, and planned behavioral health rehabilitation and treatment. Services are targeted to three specific ethnic groups: African American, Filipino, and Latino, with bilingual staff in Tagalog, Ilocano, and Spanish.

Josefa Chaboya de Narvaez BH is designated a culturally proficient site providing services to primarily the adult and older adult Latino and Vietnamese populations of Santa Clara County who have a severe mental illness. The focus of the program is to provide the services within the cultural community of the consumer utilizing appropriate intervention methods. The staff, which includes clerks, licensed social workers, psychiatrists, rehabilitation counselors, and community workers, is bilingual and/or multi-lingual in Spanish, Vietnamese, Laotian, Thai, and Chinese. The staff work in treatment teams with the consumer to provide a multi-disciplinary perspective in developing the course of treatment. In addition to the Adult/Older Adult Teams, this site also hoses treatment staff from the Family & Children's Division creating a multi-generational treatment facility.

Las Plumas Behavioral Health provides services to children, adolescents, and their families in a variety of settings including the home, school, local community, and the clinic setting. Las Plumas is one of two ABA3632 referral and assessment sites for school districts. Outpatient services include individual, group, collateral, and family therapy by licensed or licensed-eligible clinicians. Psychological testing is available when indicated, as is evaluation for psychiatric medication. Services are provided in English or Spanish by bi-cultural and bilingual staff. Cost is based on a sliding scale and ability-to-pay. Medi-Cal is also accepted. Las Plumas Behavioral Health is located in East San Jose and provides services from Milpitas to South San Jose.

Mekong Community Center provides linguistically and culturally-proficient mental health services to enable mentally-disabled Southeast Asian refugees/immigrants, particularly Vietnamese, to obtain needed skills to cope with the tasks of independent daily living and to function at their optimal level. Catholic Charities of Santa Clara County is a multi-service organization offering a broad array of services in response to community needs. Catholic Charities’ program categories include: mental health and substance abuse in a managed care division, elder care including nutrition, foster grandparent, kinship care support, mental health support services for Vietnamese and Chinese elders; housing; immigration legal services, including family visas, political asylum, deportation defense, citizenship/naturalization; refugee sponsorship and resettlement; ESL, computer classes, vocational training and employment placement and retention services for newcomers.
Managed care services include mental health and substance abuse services in a dual diagnosis context. Mental health services include a multi-disciplinary services team for older adults, children’s counseling based in 8 schools, residential foster care and clinic settings; family therapy for juvenile probation clients; vocational preparation and job placement for adults with serious mental illness. Substance abuse services include individual group outpatient treatment. A sliding fee counseling service is available for individuals and families with mental health and substance abuse issues.

Focusing on the diverse Asian American community, Asian Americans for Community Involvement (AACI)’s mission is to improve the health, mental health, and well-being of individuals and their families by providing an array of human services. AACI empowers the Asian American community by working collaboratively for equality and social justice. Founded in 1973, AACI is the largest community-based organization focused on the Asian American community in Santa Clara County. Current programs include mental health and primary care health services, HIV/AIDS testing and education, substance abuse treatment, drinking driver programs, a center for survivors of torture, programs and shelter for women and children who are domestic violence victims, a senior center, after-school youth programs, English and citizenship classes, community education, and advocacy. The Indian Health Center provides outpatient mental health and substance abuse treatment services.
