Walk-in and scheduled appointments outpatient clinic for Santa Clara County residents who are experiencing behavioral health crisis and need help.

Assisted Outpatient Treatment (AOT)

AOT services in the community include the following:
- Intensive clinical services, including individual and group
- Peer support
- Intensive case management
- Housing assistance
- 24/7 clinical crisis support

Presumptive Transfer
Santa Clara County is committed to providing mental health services to youth in out-of-home care from other counties. The Single Point of Contact for Presumptive Transfer can be reached by sending an e-mail to [email protected].
Preventative Services
In-Home Outreach Team (IHOT)

Service adults 18+ who have Medi-Cal or no insurance
IHOT mobile services in the community include the following:
- Identify and outreach to high utilizers of Emergency Psychiatric Services (EPS), Emergency Department (ED), the Criminal Justice System, and in psychiatric hospital services with Medi-Cal or no insurance
- Consult and collaborate with law enforcement
- Coordinates necessary transportation of individuals to the most appropriate level of care
- Provides Peer Support
